MOTION FOR PUBLICATION TO JURY OF AGREED JURY QUESTIONNAIRE

NOW INTO COURT, through undersigned counsel, comes Plaintiff and defendant, who respectfully seeks to utilize the attached jury questionnaire, as follows:
1.
The jury is set to be selected this date, June 7. Plaintiff brought a motion for publication of a jury questionnaire and at pre-trial conference this Honorable Court ordered that the parties confer and agree on a questionnaire or no questionnaire could be used. The parties did so and attach an agreed product of the conference. The parties seek publication to the jury and that the jurors be asked to supply responses.

WHEREFORE PLAINTIFF AND DEFENDANT PRAY THAT this motion be granted and that an appropriate order enter permitting the use of the prospective juror questionnaire in this case.
